PART 1

______ Fill out Application for Apprentice Licensure (must be typed and notarized)

______ Get certified copies of all court documents regarding your conviction: charges, convictions, sentencing orders, discharge papers

______ Fill out the “Information Regarding Felony Conviction” form. Please put your current mailing address on this form. This address does not need to match your driver's license. If this address is not correct, you will not receive communications from the Board and the review process will be delayed.

______ If you are still under supervision for your felony, have your supervisor fill out the “Monitoring Information” form. If you are no longer under supervision, provide copies of your discharge papers.

______ Submit everything to the Board office by mail:

PART 2 (where required)

______ Look in the mail for a Summary Proceeding Order Refusing to Issue License

______ Within 15 days of the date on the Summary Proceeding Order, you must make a written request for a hearing by mail, email or fax (785-296-3002).

______ Look in the mail for a Notice of Hearing explaining what date/time you should appear before the Board to explain your felony conviction. If you can’t attend, call the Board offices immediately to reschedule.

______ Attend Hearing. You may also bring witnesses or submit letters on your behalf.
